To receive a refund, the depositor submits to his agent bank only passport(identity document), and also fills out an application for payment of compensation in the prescribed form. In this case, the payment of compensation will be carried out in a non-cash manner by transferring it to an account with a bank participating in the compulsory deposit insurance system indicated by the depositor in the application for payment of compensation, or in cash by mail order at the place of residence of the depositor (except compensation on deposits (accounts) opened for entrepreneurial activities). The signature on the application sent by mail (for the amount of compensation over 3,000 rubles) must be notarized.
